I hereby declare that all the facts and information provided in the registration panel are true. I allow my personal data stated in the abovementioned applications to be processed for the purpose of e-learning platform 3DCentral, financed by the Interreg Central Europe Programme, in accordance with the General Data Protection Regulation (EU 2016/679).

Regarding my participation in the e-learning courses under the Project 3DCentral, financed by the Interreg Central Europe Programme, I hereby express consent to the processing of my personal data and dissemination of my image.

I hereby acknowledge that:

1.    the administrator of personal data shall be the office of Krakow Technology Park,  having its registered seat at ul. Podole 60, 30-394 Kraków

2.    my personal data shall be processed solely for the purpose of participation in the e-learning courses realized under the implementation of the Project 3DCentral, evaluation, control,  monitoring and reporting on the Interreg Central Europe Programme.

3.    my personal data may be made available to other entities solely for the purpose of the implementation of the Project 3DCentral, evaluation, control,  monitoring and reporting on the Interreg Central Europe Programme.

4.    the provision the said data is voluntary but the refusal to do so rules out the participation in the e-learning courses;

5.    I have the right of access to the content of my personal data and revise them.
